• Print and review the Service Plan Enrollment Form.
   
• Select your desired service plan and add it to your shopping cart, then click the check out button to enter your personal information and complete your transaction.
   
• You will receive an electronic confirmation of your purchase at the e-mail address that you provide containing your order number.
   
• Or, if you wish, simply print out and complete the enrollment form, and mail it with your payment information to:
   

• Keep a copy of Service Plan Enrollment form and your on-line purchase confirmation e-mail for your records.
   
• If you prefer to pay for this service with monthly installments, please indicate in the drop-down box labeled "Enroll in monthly payment plan" in the right-hand box above. When you choose our monthly payment plan, your credit card will be charged on the first of every month in 12 equal payments.
